What you'll do

• Perform chart reviews for all payor sources to certify appropriateness of admission and continued stay.
• Manage denied cases and refer for appeals.
• Monitor patients for progression of care and clinical readiness for discharge.
• Collaborate with SW/DCP partners to limit delays in transition of care.
• Maintain relationships with insurance companies for payment authorization.
• Educate staff regarding utilization of hospital days and resources.
