What you'll do

• Assess housing needs and maintain housing plans
• assist with housing searches, applications, and documentation
• coordinate housing deposits and authorization requests
• support move-in logistics and ongoing tenancy
• help address lease issues and prevent eviction
• connect members with community resources
• maintain EHR and case-management documentation
• coordinate with care teams and community partners.
