The Clinical Pharmacist role is onsite in San Bernardino, CA; compensation is not specified. InnovAge’s PACE program provides coordinated care intended to help older adults maintain their health and live safely at home. The pharmacist develops and implements clinical pharmacy services focused on medication management, therapy optimization, and participant goals of care. Working with primary care providers and interdisciplinary teams, the role supports safe, effective, and cost-conscious medication use and improved clinical outcomes.

What you'll do

• Provide medication consultation and education
• co-manage medication therapy with care providers
• lead medication stewardship and quality improvement
• participate in interdisciplinary meetings and medication reviews
• use participant and population data to identify medication risks
• document clinical activities and recommendations.
