Home Health Registered Nurse — North Territory

What you'll do

• Complete initial and ongoing nursing assessments, including assigned OASIS assessments
• develop, implement, evaluate, and revise individualized care plans
• provide skilled nursing procedures, treatments, medication management, education, and preventive or rehabilitative care
• evaluate patient response and continued need for services
• process physician orders
• coordinate with the interdisciplinary team and other providers
• educate patients and caregivers
• complete timely Axxess documentation
• supervise assigned LPNs and Home Health Aides
• participate in case conferences, meetings, record reviews, QAPI activities, and training
• provide on-call, weekend, or holiday coverage when assigned.

Why this role

Requires an unrestricted Illinois Registered Nurse license and current CPR or BLS certification; home health and OASIS experience are strongly preferred.
