What you'll do

• Perform assessments and procedures requiring specialized knowledge.
• Assess and review matching of employee skills to client needs.
• Visit in-home clients to evaluate quality of care.
• Review clinical notes.
• Participate in development and evaluation of the plan of care.
• Obtain physician plans of treatment and orders.
• Counsel and educate patients and families.
• Complete timely and accurate clinical notes.
• Coordinate care with the interdisciplinary team.
• Supervise paraprofessional and licensed staff.
