Description

Supervise and train primary care providers (PCPs). Maximize core model execution, including clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Provide direct patient care as a PCP and/or other clinical duties. Lead care team consisting of medical assistant, care facilitator, and care coordinator.
