What you'll do

• Assess and evaluate patients and collaborate with the clinical team on care plans
• administer medications, injections, and treatments and perform routine procedures
• provide telephone triage, health counseling, and patient education
• review physician orders and lab requests and follow up
• measure and record vital signs
• communicate test results as directed and document in the EHR
• assist with forms and patient calls
• support chart preparation and care-gap follow-up
• maintain confidentiality and an organized work area.
