What you'll do

• Assess, plan, implement, and evaluate care needs of patients.
• Provide nursing care and clinical support tasks.
• Coordinate patient self-management initiatives in a family-centric program.
• Facilitate improving self-management age-specific goals.
• Provide lifestyle coaching and conduct patient visits.
• Provide patient education and collaborate with outside community resources.
• Provide case management including oversight and interpretation of laboratory results.
• Order therapeutics, durable medical supplies, and prescription refills.
• Coordinate referrals and diagnostic studies.
• Function as an expert clinician and diabetes educator.
• Participate in performance improvement initiatives.
• Evaluate and integrate evidence-based practice into clinical practice.
• Conduct and document accurate clinical assessments.
• Develop, implement, and evaluate the patient’s individualized plan of care.
• Manage patient assignments by delegating effectively.
• Develop a comprehensive educational plan for the patient and family.
• Recognize patient/family rights.
• Continually evaluate and revise the care plan.
• Develop a comprehensive discharge plan.
• Support the development of new staff members.
• Guide professional growth of department staff.
• Participate in departmental committees.
