What you'll do

• Lead and mentor supervisors, social workers, behavioral health specialists, and assistants through hiring, training, supervision, and coaching. Oversee social work assessments and team participation
• ensure interventions align with patient care plans and urgent needs
• maintain compliance in documentation, assessments, care planning, reviews, patient rights, and grievance processes
• collaborate on support groups, counseling, family education, and community partnerships
• support end-of-life care planning and coordination
• conduct home visits as needed.
