What you'll do

• Collect and document patient health information, prepare patients for clinicians, and implement care plans.
• Provide nursing visits, medication administration and education, injections, wound care, specimen collection, basic laboratory tests, and other listed clinical services.
• Conduct phone triage, document patient encounters, coordinate prescription requests and prior authorizations, maintain a safe clinical environment, and communicate with the care team.
