Hospice Registered Nurse (RN) Case Manager

• Perform and coordinate field visits with the nursing team according to patient care plans
• educate patients, families, caregivers, and staff about medications, durable medical equipment, supplies, and disease progression
• address concerns and support patient comfort
• communicate hospice philosophy, pain management, and terminal-illness needs in understandable language
• demonstrate agency values and strong interpersonal communication.
