Registered Nurse (RN) Case Manager — Hospice is a field-based, full-time role in Bloomington, MN. Pay is $75,000–$90,000 per year. What you'll do

• Complete initial and ongoing assessments
• develop and update individualized plans of care
• provide skilled nursing, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among families, physicians, and the care team
• participate in interdisciplinary group meetings.
