Occupational Therapist (Early Intervention)

Provide outreach services to children and families. Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients. Maintain client records completely and accurately to include IFSP’s with full assessment of client functioning and goals. Documentation of client progress in the program, records of required contact with collaterals and family
