What you'll do

• Coordinate and monitor services
• conduct visits with individuals and Family Model Providers
• ensure support plans, authorizations, and goals are followed
• maintain documentation and reports
• monitor staff and provider performance
• coordinate admissions and transitions
• ensure regulatory compliance
• follow up on incidents and complaints
• monitor scheduling and electronic visit verification
• support referrals and provider development
• participate in an after-hours on-call rotation.
