What you'll do

• Provide care according to an individualized plan in coordination with a registered nurse and other clinicians
• record patient health status and report it to the nurse
• assist with dressing, bathing, grooming, toileting, eating, mobility, transfers, and incontinence care
• prepare meals and snacks
• perform light housekeeping
• assist with transportation needs.
