Case Coordinator

What you'll do

• Develop individual programs and ensure they meet participant needs.
• Assist participants in understanding program goals.
• Act as a liaison for families.
• Organize and chair participant conferences.
• Review reports and ensure appropriate documentation.
• Coordinate discharge planning and internal medical appointments.
