The coordinator supports individuals receiving outpatient medication management by monitoring medication effectiveness and side effects, coordinating services, and maintaining treatment documentation. The role works with psychiatric providers and helps individuals, families, and caregivers build knowledge that supports independence and quality of life.

What you'll do

• Complete initial and ongoing assessments
• monitor mental stabilization through regular contact
• coordinate treatment-plan services and community referrals
• maintain contact during medication appointments and by phone as needed
• advocate for individuals’ rights
• assess risk factors and report concerns according to protocol
• maintain documentation and consents
• and develop and update treatment plans with individual participation.
