Home Care Coordinator

Job overview

What you'll do

• Schedule HHAs and PCAs across NYC boroughs
• manage Medicaid and MLTC-authorized cases and hours
• maintain Plans of Care
• liaise with clients, families, caregivers, and MLTC case managers
• monitor EVV
• maintain HIPAA and New York State privacy compliance
• handle call-outs, emergency coverage, and schedule changes
• maintain audit and inspection documentation
• verify caregiver credentials and training
• address client concerns and incident reports.
