What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine eligibility and appropriate levels of care
• develop and document plans of care
• educate patients and families on self-care, nutrition, and dietary needs
• provide preventative and rehabilitative nursing procedures
• apply infection-control precautions
• direct Home Health Aides and LPN/LVNs
• document progress
• communicate changes to physicians and care team
• monitor cases for third-party payer compliance.
