What you'll do

• Identify and coordinate patient and family care, with contacts at least every 15 days
• educate patients and families about disease progression, hospice philosophy, and end-of-life changes
• review caseload needs and make referrals
• administer prescribed medications and treatments
• lead interdisciplinary care group planning
• document care in the electronic health record
• assess treatment response and revise care plans
• supervise home health aide and licensed practical nurse visits
• support discharge planning and continuity of care.

Why this role

Requires a state-issued Registered Nurse license, Basic Life Support certification upon hire unless waived by the department leader, a valid driver’s license, and completion of an accredited nursing program. Typically requires 1–2 years of relevant nursing experience. Schedule is Monday–Friday, 0800–1630; one to two holidays per year. After-hours, weekend, and on-call opportunities are available but not required.
