What you'll do
- Coordinate the Resident Assessment Instrument process and care management from admission to discharge
- schedule MDS assessments and care-plan reviews
- monitor documentation and maintain current MDS status
- enter resident information into MDS programs and code assessments using records, observation, and interviews
- attend interdisciplinary, quality-assurance, and payer meetings
- support updates following significant changes
- prepare care-planning conference notices
- submit completed assessment data to government agencies
- instruct staff on related assessment and clinical computer processes.
