Medical Case Manager - Housing

The Medical Case Manager - Housing is an onsite role in Rock Hill, SC, with occasional in-person work in the community or at patients’ homes. Pay is $23.00–$33.00 per hour, based on qualifications and experience. Affinity Health Center’s case management services support people living with HIV/AIDS in its service area. The role combines housing-resource coordination with medical case management to help patients address housing needs, access benefits and services, and remain connected to medical care. It also supports accurate eligibility documentation, program reporting, and compliant delivery of services under Ryan White guidelines.

What you'll do

• Research housing programs and maintain relationships with landlords and partners
• provide housing-resource and eligibility information
• support the MCM team with eligibility direction and compliant chart documentation
• assist with chart audits and data review
• prepare reports for funders and partners
• coordinate subsidy documentation with Accounting
• manage a caseload of up to 50 patients per year
• assess patient needs and eligibility
• develop action plans
• coordinate referrals and support applications for public programs
• document services and participate in quality improvement.
