The Medical Assistant (Health Navigator) — Senior Health role is onsite in Surprise, AZ. Compensation is $23 per hour based on a full-time schedule. One Medical provides primary care services, and this office is presently focused on its Senior Health member population, age 65 and older. The health navigator supports a primary care team by building patient relationships, collecting health information, coordinating follow-up care, and helping patients navigate preventive, chronic, and acute care needs. The role contributes to an accessible, supportive in-office experience and better healthcare outcomes.

What you'll do

• Room patients, capture relevant health information, take vitals, administer point-of-care testing, and perform standard screenings
• schedule specialist appointments and coordinate referrals
• share information with internal and external care teams
• collaborate with providers to monitor preventive care
• support daily practice operations, including calls, emails, front desk inquiries, and clinical space upkeep
• assist with patient education on chronic disease management and action planning.

Why this role

Full-time schedule is 40 hours per week, Monday–Friday, 8 AM–5 PM. Requires at least six months of healthcare patient-facing or high-touch customer service experience and qualifying Medical Assistant education, training, or certification.
