What you'll do

• Provide individualized, stage-matched support using motivational and strength-based approaches
• help clients pursue recovery goals
• coordinate mental health, physical health, and substance use services
• build daily living and symptom-management skills
• connect clients with healthcare, benefits, housing, education, and employment resources
• assist with crisis planning and discharge coordination
• facilitate or co-facilitate groups
• collaborate with medical and clinical providers
• provide transportation support and accompany clients to appointments when needed.
