What you'll do

• Assess patients and collaborate with the care team to develop and implement care plans
• make referrals based on identified needs or document why a referral is not made
• teach self-care and support rehabilitation
• supervise LPNs and HHAs
• complete timely, accurate documentation
• communicate patient progress to the clinical manager and care team
• plan and provide staff education
• perform assigned on-call responsibilities
• participate in clinical development and continuing education.
