What you'll do

• Collaborate with DCFS caseworkers to assess youth needs and coordinate services
• support care-plan development and implementation
• coordinate and follow up on referrals
• participate in Child and Family Team meetings, court hearings, and Administrative Case Reviews
• identify service gaps or delays
• maintain confidential records and track outcomes
• support incident response
• follow DCFS rules and youth rights
• participate in required training
• support caregiver orientation and staff training
• coordinate with behavioral health, education, and housing teams.
