The role supports safe, high-quality care through assessments, patient education, condition monitoring, plan-of-care coordination, and timely documentation.
What you'll do
- Complete comprehensive OASIS assessments and Start of Care and other skilled nursing visits
- reconcile medications and educate patients and caregivers
- support disease-process and chronic-condition management
- assess and treat wounds when ordered
- monitor changes in patient condition and communicate significant findings
- update and coordinate plans of care
- complete accurate and timely clinical documentation
- coordinate with physicians, therapists, and the interdisciplinary team.
