What you'll do

• Assess treatment needs and develop, monitor, and evaluate continued care plans
• document patient interactions and discharge plans in the EMR
• coordinate referral options and outside assessments
• obtain and upload releases of information
• assist with initial assessments, legal issues, FMLA, food stamps, external appointments, discharge planning, and medication-assisted treatment
• participate in crisis intervention situations.
