The nurse evaluates patients’ needs and delivers skilled care under individualized plans of care, supporting patient well-being through clinical visits, coordination with physicians and other clinicians, and communication with patients and families.

What you'll do

• Provide nursing services according to the plan of care
• conduct initial evaluations and regular reassessments
• deliver specialized, preventative, and rehabilitative nursing care
• document visits and care conferences
• report changes in patient condition
• counsel patients and families
• process orders and complete certification, recertification, and discharge documentation
• coordinate referrals
• supervise and teach nursing personnel
• participate in care conferences, peer review, and performance improvement.
