What you'll do

• Provide counseling, case management, and care coordination
• develop individualized strength-based treatment plans
• deliver community outreach
• work within the multidisciplinary PACT team and fidelity standards
• conduct assessments, risk evaluations, and safety planning
• help clients access resources and navigate care systems
• coordinate with providers and agencies
• facilitate groups and psychoeducation as needed
• maintain timely documentation
• participate in meetings, supervision, and training.

Why this role

Requires a BA/BS/BSW in a behavioral science-related field, Washington State driver’s license, insurance, reliable transportation, and age 21 or older. Agency Affiliated Counselor registration must be completed if not already credentialed. Includes a 24/7 on-call rotation and evenings, weekends, and holidays as assigned.
