The Remote Care Coordinator, Cardiac Care position is 100% remote, with candidates in or familiar with the Dallas/Fort Worth region preferred. The posting lists pay of $26–$32 per hour and a Monday–Friday schedule of 40 hours per week during flexible business hours. Cardiac Care Alliance works with independent cardiologists to support value-based care and improve patient access, outcomes, and experience. The coordinator provides proactive, non-clinical support to patients with complex cardiac conditions, primarily congestive heart failure. Through phone outreach, follow-up, education, and care-management documentation, the role helps patients stay connected to care and escalates concerns to registered nurses and cardiologists as appropriate.

What you'll do

• Conduct structured phone outreach
• manage an assigned patient caseload using risk stratification
• complete initial assessments and follow-ups on symptoms, medication adherence, functional and psychosocial status
• identify social needs and escalate resource needs
• support care planning and specialty follow-up
• review population-health dashboards for care gaps
• provide patient education
• monitor for worsening conditions and escalate concerns
• support post-discharge follow-up within 48 hours
• document interventions, care plans, and goals
• communicate with RNs, cardiologists, primary care offices, and clinical partners.
