Regional Hospital Liaison

The Regional Hospital Liaison role is onsite in Cleveland, OH; compensation is not specified. The liaison represents Garden Springs Healthcare’s facilities across the Greater Cleveland hospital market and works with facility leadership, clinical teams, Centralized Admissions & Intake, and corporate business development. This role builds hospital and referral-partner relationships and helps coordinate appropriate post-acute placements. It supports referral conversion, admissions, and census growth across the network.

What you'll do

• Develop relationships with hospital case managers, social workers, discharge planners, physicians, utilization review teams, and other referral sources
• represent the network’s clinical capabilities and placement options
• identify account growth opportunities
• support complex discharge planning and placements
• coordinate with admissions and facility teams
• conduct bedside visits and patient/family meetings when needed
• track referral patterns and participate in hospital and community business development activities.
