Consultant Pharmacist, PACE (Part-Time)

The Consultant Pharmacist, PACE (Part-Time) role is onsite in Alpena, MI, paying $61–$67 per hour. The schedule is one day per week. Kalamazoo Long Term Care Pharmacy supports a PACE center serving older adults who aim to remain safely in their homes and communities. As the on-site pharmacy expert, the pharmacist contributes to the interdisciplinary care team through medication management, safer prescribing, and coordination between the center and pharmacy. The work helps address medication risks and support participants’ therapy and care transitions.

What you'll do

• Conduct comprehensive medication reviews and reconciliations at enrollment, care transitions, and after hospital stays. Participate in care planning meetings and reassessments with the interdisciplinary team. Collaborate with clinicians on medication issues and evidence-based prescribing
• identify polypharmacy, high-risk medications, and deprescribing opportunities. Liaise with the pharmacy on refills, deliveries, and medication administration record issues
• oversee on-site medication storage, inventory, and emergency supplies
• educate staff, participants, and caregivers
• support formulary management, quality improvement, and regulatory compliance.

Why this role

One day per week onsite at a PACE center. PharmD and an active, unrestricted Michigan pharmacist license—or eligibility to obtain one before starting—are required. Geriatric, PACE, or long-term care experience and BCGP certification are preferred, not required.
