PRN Home Health Registered Nurse — Client Assessments

The PRN Home Health Registered Nurse — Client Assessments role is field-based from Mentor, OH, covering Lake, Lorain, Cuyahoga, Medina, and Geauga counties. Pay is $56 per completed visit, with mileage reimbursement of $0.58 per mile after the first 10 miles driven to the first patient of the day. The nurse supports Better at Home’s home health team. The nurse conducts in-home assessments and reassessments, develops individualized care plans, and documents findings in accordance with state and Medicaid guidelines. The role helps determine patient needs and program eligibility while coordinating with clinical supervisors, physicians, and care team members to support continuity of care.

What you'll do

• Conduct initial in-home assessments, routine reassessments, and recertifications for private-pay and Medicaid waiver patients
• develop, implement, and update individualized plans of care
• travel across the five-county service area
• maintain compliant electronic medical records
• coordinate with clinical supervisors, physicians, and care team members.

Why this role
