What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• ambulate and transfer patients as appropriate
• assist with range-of-motion exercises and nutritional snacks
• assist with pre-poured, ordinarily self-administered medications under nurse supervision in home care only
• perform simple wound, skin, and bowel care where appropriate
• report changes in patient status
• document care and observations
• attend care conferences and meetings
• participate in scheduled on-call rotation for home care only.
