What you'll do

• Act as a clinical liaison with outside agencies
• Prepare reports and develop protocols
• Act as a patient advocate and educator
• Develop individualized patient/family education plans
• Facilitate patient return to daily activities
• Consult with internal and external providers
• Ensure accurate records are maintained.
