What you'll do

• Develop, plan, implement, and evaluate the Social Services program in accordance with applicable guidelines
• identify and address residents’ social, emotional, and psychological needs
• coordinate adjustment, care planning, and discharge services
• collaborate with interdisciplinary teams
• assist residents and families with resources, education, and transitions
• maintain regulatory documentation, including MDS-related processes
• advocate for residents’ rights
• support Medicare, Medicaid, reimbursement, and eligibility compliance.
