Planned Care Coordinator

What you'll do

• Develop and deliver training on quality improvement workflows, population health, Epic EHR, and patient engagement
• educate patients and caregivers

• coordinate outreach for screenings, immunizations, and wellness
• monitor training and quality metrics
• analyze population health data and prepare reports
• collaborate with clinical and community teams
• coordinate referrals and follow-up
• document patient interactions and care coordination in the EHR.
