Nurse Case Manager – Valor House

What you'll do

• Supervise the medication control system in accordance with the Utah Nurse Practice Act
• conduct medical needs assessments and develop care plans with the interdisciplinary team
• coordinate referrals and appointments with outside providers
• assist with at-home medical procedures within nursing scope
• provide health education
• document nursing contacts and medication events in the electronic medical record
• participate in quality improvement, case consultations, and team meetings.
