What you'll do

• Complete comprehensive member assessments within regulated timelines, including home visits as required
• facilitate waiver enrollment and disenrollment
• develop and implement care plans with members and their support networks
• monitor care plans and document interventions and outcomes
• coordinate behavioral health, long-term services and supports, and community resources
• assess medical necessity and authorize appropriate waiver services
• evaluate covered benefits and funding sources
• facilitate interdisciplinary care team meetings
• use motivational interviewing to educate and support members
• address barriers to care
• identify critical incidents and develop prevention plans
• collaborate with care managers and leadership.
