What you'll do

• Collect participant assessment data
• collaboratively set educational goals, learning objectives, content, and teaching methods
• provide interactive, collaborative, skill-based education
• develop individualized follow-up plans
• evaluate learning objectives and conduct follow-up assessments
• document assessments, plans, services, and results
• collaborate with the care team
• identify needs outside the instructor’s scope and arrange additional services
• contribute to training materials and continuous quality improvement
• participate in peer review.

Why this role

Requires an active Registered Dietitian registration or Registered Nurse registration with the California Board of Registered Nursing, a related associate’s or bachelor’s degree, and current CDCES certification or eligibility to obtain it within two years. At least six months of diabetes education experience in group and one-to-one settings is required. Bilingual ability may be required at certain clinics or shifts.
