What you'll do

• Provide hospice nursing care in patients’ homes and other care settings
• complete nursing assessments and individualized care plans
• manage symptoms and monitor changes in condition
• coordinate medications, treatments, supplies, and other needs
• educate patients and families about medications, symptom management, disease progression, and end-of-life care
• communicate with physicians and interdisciplinary team members
• participate in interdisciplinary group meetings
• provide emotional support
• and maintain timely clinical documentation.
