Nephrology Nurse Navigator

Job overview

The Nephrology Nurse Navigator is an onsite, full-time role in Peoria, IL, with pay of $35–$45 per hour. The navigator works with RenalCare Associates’ nephrologists, clinical teams, dialysis centers, and healthcare partners to support patients with chronic kidney disease. The registered nurse educates patients and families, coordinates referrals and follow-up, tracks care milestones, and helps address barriers to timely care. The role provides a consistent point of contact as patients consider treatment options and move through care transitions.

What you'll do

• Educate patients and families about chronic kidney disease, progression, and treatment options
• explain dialysis modalities and transplantation
• coordinate transplant evaluations, dialysis access planning, and specialized referrals
• collaborate with nephrologists, clinic nurses, dialysis centers, and other providers
• track milestones, labs, referrals, and treatment planning
• identify barriers and connect patients with resources
• document and communicate care updates.
