What you'll do
- Assist physicians and nurse practitioners with medication management, service plans, and hospital discharge clinics
- conduct routine screenings for therapeutic and physical health services
- consult with staff
- complete required clinical documentation
- ensure patients are seen routinely before medication refills
- monitor changes in mental status or medical conditions
- collaborate with staff and interdisciplinary teams
- and handle medication orders according to policies and regulations.
