The role follows individualized service plans, documents services and outcomes, and helps members work toward personal goals.

What you'll do

• Review and document shift communication logs
• use members’ crisis plans, safety plans, and coping skills
• supervise member activities and safety
• assist with cooking, cleaning, and organizing
• provide skill development services according to service plans
• transport members to assigned destinations
• complete progress notes and incident documentation
• attend required training
• obtain medication management certification
• administer and document medication
• and maintain confidentiality.

Why this role
