The position is part of CommonHeart’s Home Health and Hospice team, providing end-of-life care to patients and families. The nurse provides skilled, compassionate hospice care, collaborates on individualized care plans, and supports patients and families through education, comfort, and advocacy. Documentation and communication about changes in condition help the interdisciplinary team coordinate care.

What you'll do

• Provide skilled nursing care to hospice patients
• collaborate with an interdisciplinary team to develop and implement individualized care plans
• educate and support patients and families
• document care and communicate changes in condition. The role includes on-call coverage, up to one shift per week Monday–Thursday from 4 PM to 8 AM and up to every fourth weekend from Friday 4 PM to Monday 8 AM.
