What you'll do

• Build relationships with senior living communities, active adult neighborhoods, employers, healthcare providers, community organizations, and civic leaders
• attend community meetings and support educational and wellness events
• connect people to care, providers, services, and resources
• identify barriers and facilitate referrals
• coordinate healthy-aging, prevention, and wellness programming
• monitor participation, referrals, engagement, and outcomes
• use data and stakeholder feedback to recommend strategies
• collaborate with MUSC leaders, practices, service lines, and teams.

Why this role

Requires a bachelor’s degree in a clinical healthcare discipline, current South Carolina licensure or certification in an approved clinical discipline, at least five years of relevant experience, a valid driver’s license, reliable transportation, and frequent travel throughout the assigned service area.
