What you'll do

• Lead and manage the social services department
• complete psychosocial assessments and care plans
• participate in interdisciplinary care planning
• coordinate discharge plans and referrals
• support residents and families with adjustment, grief, behavioral concerns, and end-of-life planning
• assist with advance directives, health care proxies, guardianship, and MassHealth applications
• investigate grievances
• support resident rights and abuse prevention and reporting
• maintain compliant documentation
• support survey readiness and QAPI initiatives.

Why this role
