What you'll do

• Maintain resident information related to care plans, MDS, participation records, progress notes, and reviews
• complete initial activity assessments
• participate in care-plan conferences and interdisciplinary meetings as required
• plan, lead, and assist with group activities and resident committee meetings
• monitor and improve programs
• coordinate with departments and volunteers
• protect confidentiality and resident rights
• respond appropriately to emergencies and changing resident needs.

Why this role
