The Certified Nursing Assistant (CNA) — Memory Care role is onsite in Rockford, IL. Pay is $19.50–$26.00 per hour. The role supports the Memory Care Unit at Alden Debes Rehabilitation and Health Care Center. The CNA provides person-centered support to residents with dementia, helping maintain their quality of life through daily care, documentation, safety, and collaboration with the nursing team. The role also supports individualized activities and care approaches that respond to residents’ needs.

What you'll do

• Assist with activities of daily living, restorative programs and exercises, and documentation of resident activities and functions.
• Support the Memory Care Program with person-centered activities, resident-specific communication and symptom-management approaches, a safe environment, family involvement, and required documentation.
• Provide emotional support and feedback to the Memory Care Director and collaborate with the interdisciplinary team.
